We work closely with you - and where appropriate, your GP, district nurses, occupational therapists, physiotherapists, dietitians, social workers, and other professionals, along with people who are important to you, family friends and support givers - to build a complete picture of your health and support network.
Our approach focuses on what you can do, identifying how we can support you to carry out daily living activities safely, confidently, and with dignity.

Our new client assessment is a vital first step in creating a personalised care plan tailored to your individual needs, preferences, and lifestyle.
Supporting you in your own home and out in the local community with dignity, respect, and personalised care.
Our comprehensive assessment for all our clients, allows us to build a tailored care plan that promotes your independence, respects your choices, and helps reduce risks such as falls, illness, or loss of independence.

We take time to get to know you as a person—your background, interests, values, preferred routines, communication style, and important people in your life.

We review your medical history and how it may affect your daily life, safety, and independence. Looking at what’s important to you to maintain a good quality of life and remain in the comfort and familiarity of your home and local community.

We assess any mobility aids or equipment used to ensure safe and effective moving and handling support. Working along side Occupational therapists and physios to build a safe environment and moving and handling support plan.

This includes your preferences around personal hygiene and care routines, skin care routines, toileting needs and any relevant history to enable us to promote your dignity.

We discuss the level of support needed, and document current medications, including topical creams and shop bought medications and creams.
We lease with GP’s and pharmacists as needed to help ensure safe management of medication and prescriptions.

We assess your home to identify any risks or barriers to safe care delivery. This includes checking where key utilities and safety items (like fuse boxes, water stopcocks, smoke alarms) are located and if pets in the home how they are supported, to help keep you and your care support safe.

We note your dietary and hydration needs and preferences to support nutrition and well-being.
We work with dietitians, diabetes nurses and GP’s to ensure we are supporting with dietary and hydration needs where appropriate.

Not being able to comfortably assess your local community can have an isolating impact on you. We like to discuss what support you have to access your community, shopping, banking, socialising.
